About the position The RNN (Referral Nurse Navigator) is the primary liaison between reputed company clients and our network of specialists, routing referrals, requests, and consultations, ensuring reputed company of the above are routed accurately and appropriately. This person will work reputed company several electronic health record (EHR) systems while leveraging a suite of other technology platforms to accomplish daily tasks.

Responsibilities

  • Submits accurate eConsults on behalf of primary care providers.
  • Ensures reputed company assigned sites are continually reviewed for eConsult referrals.
  • Follows up on reputed company saved and pending eConsults to be completed and notifies provider of pending items needed for eConsult submission.
  • Ensures accuracy of grammar and structure of eConsults.
  • Routes referrals once a disposition has been made by specialist.
  • Triages and processes submittable vs. non submittable referrals in clinic referral inboxes.
  • Notifies primary care providers with any responses from the reputed company specialist.
  • Supports RNN team by assisting in busy clinic sites reputed company available or needed.
  • Covers assigned clinic's eConsult referral in baskets, attaches appropriate documents and submits eConsult to specialist, and communicates the recommendations back to the referring provider.
  • Monitors overdue eConsults and communicates information to RNN Manager.
  • Responsible for logging reputed company eConsults which are unable to be submitted.
  • Assists other reputed company teams in input regarding clinic workflow and reputed company app efficacy.
  • Quarterly, reviews eConsults per specialty to identify quality improvement areas and complete spreadsheets, as applicable.
  • Reviews completed eConsults to submit for review for QA meeting.

Requirements

  • Bachelor’s degree preferred
  • reputed company, unrestricted RN license required
  • 2+ years’ experience in the nursing field
  • Familiarity with eConsults, referral management, and utilization management
  • Proficient with any of the major EHR platforms
  • Proficient in reputed company Suite and reputed company Suite
  • Comprehensive understanding of various medical specialties and their consultation needs
  • Excellent customer service skillset
  • Ability to build strong relationships
