About us Francis Supported Living Services is a 24-hour accommodation service based in Chatham in Kent. We specialise in supporting young people (16 - 17) year olds who may face challenges with learning disabilities such as Mild to Moderate Autism Diagnosis Learning disabilities Job Title Support Worker - 16-17 Years Olds (Weekends Only) Responsibilities and Outcomes Skills / Experience Essential Key Responsibilities 1. Assessing resident needs through robust Support Planning and Risk Assessments and obtaining necessary information from other agencies. 2. Ensure that resident support plans and risk assessment are followed and contribute to regular reviews of these plans, for individuals to achieve the greatest possible control over their own life. 3. Assist residents to manage and maximise their finances effectively. 4. Ensuring that all residents’ records are accurately maintained. 5. Promote independent living, confidence building, self-awareness, training, education, employment opportunities and participation in the local community. 6. Enable residents to develop and implement their support plan needs and risk assessments. 7. Communicate/consult with relatives, partners, advocates, friends or carers and other staff and agencies, as appropriate, regarding the resident’s health, social care, and well-being. 8. Promote resident involvement in shaping and monitoring services, so they have real choice and control over the service they receive. 2 9. Provide high quality, responsive and person-centred services that meet the physical, mental, and emotional needs of residents and ensuring people have choice and control over their own lives. 10. Be aware of and follow Prosper Housing Group policies and procedures, with particular attention to standards of resident service, health and safety, and equality and diversity. 11. Demonstrates a sensitive and supportive approach to vulnerable people maintaining their dignity and self-respect at all times. 12. Knowledge of welfare benefits. 13. Good written and oral communication, numeracy, IT and interpersonal skills. 14. Knowledge of organisations providing services to vulnerable people. 15. Ability to cope with verbally aggressive and volatile situations. Desirable 16. Understanding of Health and Safety issues. 17. Experience of supported housing work including housing management, advocacy, and debt management. 18. Be able to cover shifts at short notices and flexible in a demanding working environment Qualifications 19. Appropriate professional or vocational qualification – desirable. NVQs 2-3 Key Outcomes 20. Residents achieve independent living. 21. Activity programmes within the scheme or community where appropriate. 22. Appropriate support plans are in place and implemented. 23. Residents’ finances are managed with support. PART B For Management Roles (including Senior SW) only Planning and achieving results 24. Plans on a daily basis and prioritises routine tasks under guidance. 3 25. Accountable for completing tasks according to standards following policies procedures and protocols. Resident Service 26. Thinks of simple ways of dealing with other’s needs. 27. Initiates action to resolve resident problems, responding to the needs and circumstances of the resident. 28. Follows up to ensure that the service delivered meets the needs of the resident, keeping promises and commitments. 29. May support external residents with requests for information within guidelines, routine needs or simple problems. Problem solving and decision-making 30. Deals with a range of basic problems as they occur, responding to Immediate operational needs. 31. Takes decisions within the context of a routine job environment. 32. Chooses from appropriate options in response to work issues that arise, seeking advice and assistance in response to more complex issues. Knowledge, skills, and experience 33. Work involves a variety of tasks that follow established sequences or procedures. 34. Work requires basic skills acquired through training and work experience to an equivalent of a vocational or academic qualification at level NVQs 2-3. People Skills 35. Works cooperatively with others, building relationships in own area of work to deliver priorities. 36. Communicates straightforward information, noting information and requests accurately, seeking clarification where needed. 37. Uses language others will understand, adapts behaviour and takes reasonable steps to understand and respond to people with different backgrounds and needs. 38. May produce and make alterations to standard documentation within an established framework. 39.
