In-Home Physical Therapist
Job Overview We are seeking a dedicated and compassionate In-Home Physical Therapist to join our dynamic healthcare team. - Conduct comprehensive assessments of patients’ physical condition, including mobility, strength, and functional limitations
- Develop and implement tailored care plans that align with patients’ goals and medical needs
- Administer physical therapy treatments per MD protocols
- Monitor patient progress regularly, adjusting care plans as necessary to optimize outcomes
- Educate patients and caregivers on injury prevention, exercise techniques, and home safety strategies
- Document all patient interactions accurately
- Collaborate with our case managers and physicians/nurses to ensure comprehensive patient care
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to Orthopedic practice
- Proven experience in home health care, outpatient clinics, hospitals, or inpatient settings with a focus on patient care excellence
- Familiarity with physiotherapy techniques such as trigger point therapy and orthopedics treatment methods
- Ability to interpret medical reports and collaborate effectively within multidisciplinary teams
- Excellent communication skills to educate patients and support their recovery journey
