Job Description

Summary: Schedule: Monday - Friday, No Weekends and No Holidays - 9:00am - 5:30pm - Looking for flexibility to pick-up 3-4 shifts a week per business need We are seeking experienced Registered Nurses to join a dynamic and fast-paced call center supporting over 68 primary care offices and several PACT model practices. In this role, you will be the first point of contact for patients experiencing emergent symptoms, providing expert clinical guidance over the phone. Your ability to assess situations quickly, ask the right questions, and use sound clinical judgment will help ensure patients receive the appropriate level of care. As part of our nurse triage team, you will manage a high volume of calls and MyChart messages, working efficiently within CareConnect to document and communicate patient needs. This position requires strong multitasking skills, excellent listening abilities, and a commitment to delivering exceptional customer service. While you won’t make diagnoses, you will use your clinical expertise to guide patients safely and effectively, representing their primary care provider with professionalism and empathy. You’ll be part of a supportive, collaborative team that values connection and growth. This position provides nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team. The RN is accountable for the practice of nursing as defined by the Ohio Board of Nursing. The RN delegates nursing tasks to licensed practical nurses (LPNs) and unlicensed assistive personnel (UAPs) using the criteria written in the Ohio Nurse Practice Act.

Responsibilities

And Duties: 30% Assessment/Diagnosis Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual. 30% Outcomes Identification/Planning Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs. 20% Implementation/Evaluation Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data. 10% Leadership - Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance. 10% Operations As a High Reliability Organization (HRO), responsibilities require focus on safety, quality and efficiency in performing job duties.
