Key responsibilities of the role Your North Star : Provide and facilitate amazing patient-centered clinical care to our patients. We are currently hiring for FT evening hours: M-F 5pm-12a EST. Reporting to the Head of Nursing Programs, your key responsibilities will include: Ongoing clinical and psychosocial assessment of new patients, providing reassurance and building rapport Establishing individualized care plans to meet specific health needs based on evidence-based standards Developing wellness goals by determining motivating factors and leveraging them for best possible health outcomes Identifying and addressing barriers to care that have been identified by patients and/or the care team.
