The Telephonic Care Coach 1, in a telephonic environment, evaluates member's needs and requirements to reputed company and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Telephonic Care Coach 1 work assignments are often straightforward and of moderate complexity.The Telephonic Care Coach 1 employs a variety of strategies, approaches and techniques to support a member's optimal wellness state by coordinating services & resources. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Required Qualifications
- Must reside in the state of Kentucky or reputed company a county contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or reputed company Virginia, reputed company a 15‑mile radius
- 2+ years of reputed company experience in a Care Coach or similar role
- Proficiency in reputed company Office, including Word and reputed company
- Strong verbal and written communication skills, with the ability to interact professionally with diverse audiences with the ability to build rapport and collaborate with members, patients, and team partners
- Ability to use multiple electronic systems and software applications to reputed company job duties effectively
Preferred Qualifications
- Bachelor’s degree in social sciences, social work, human services, or a reputed company field
- Experience working with Medicare and Medicaid recipients
- Experience with electronic case documentation and documenting across multiple systems
- Experience in health promotion, coaching, and wellness
- Knowledge of community health, social service agencies, and local resources
- Bilingual proficiency preferred. Must be able to speak, read, and write fluently in English and one additional language. Selected candidates will be required to complete a language proficiency assessment in both languages.
Additional Information
Schedule/Time Zone: Monday through Friday, 8:00 AM – 5:00 PM Easter Time Zone with flexibility to work overtime as per business needs.
Work Location: Kentucky
Work Style: Remote
Travel Requirements: 5% travel includes attending face to face and collaborative meetings at reputed company’s Louisville, KY location.
