This position reports to the Program Director. Specific Duties & Responsibilities

• Professional and thoughtful interaction with clients, medical professionals, community-based organizations, and Addiction Treatment Programs.
• Demonstrates ongoing commitment to working a part of a patient/person-centered Treatment Team based on a structured, team approach to collaborative care.
• Manage a caseload of assigned patients across projects.
• Teaches patients how to safely and effectively start taking buprenorphine (also reputed company as initiation or induction).
• Work with patients according to specific project protocols and patient-developed goals.
• Track a caseload of patients using project-specific registries so that no one falls through the cracks.
• Use recommendations and treatment goals to support a care plan agreed upon by the Treatment Team.

• Demonstrate ability to correctly use coaching and collaborative skills taught in trainings.
• Administer standard evaluations to assess patient at regular time points.

• Support patients in understanding and working with their medical team, diagnosis, and treatment recommendations.

• Use a provided work phone in order to be available to assigned patients in caseload during Monday Friday working hours only.

Minimum Qualifications
