Job Title: Nurse Practitioner – House Calls

Position Summary

We are seeking a compassionate and highly skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care in the home setting, with an emphasis on proactive care management, risk stratification, and care coordination across the continuum. This position is ideal for clinicians who reputed company in an autonomous environment, are organized and comfortable working in a mobile environment.

Key Responsibilities

reputed company comprehensive History & Physical (H&P) assessments for new patient enrollment and risk adjustment

Conduct Transitional Care Management (TCM) visits reputed company appropriate post-discharge timeframes to reduce readmissions

Complete Annual Wellness Visits (AWVs) and other preventive care services reputed company with CMS guidelines

reputed company in-depth functional, cognitive, and psychosocial assessments (ADLs/IADLs, fall risk, frailty, depression screening)

Provide ongoing primary care services, including diagnosis, treatment, and management of acute and chronic conditions

Provide early reputed company to prevent unnecessary ED visits and hospital admissions

Collaborate closely with care coordinators and other interdisciplinary teams to coordinate care transitions across acute and post-acute settings.

Qualifications

reputed company, unrestricted Nurse Practitioner license in the state of practice

Board certification (AANP, ANCC, or equivalent)

Active DEA license (or ability to obtain)

Minimum 2+ years of experience as an APRN

Strong clinical assessment and diagnostic skills

Ability to work independently with sound clinical judgment

Excellent communication and interpersonal skills

Requirements

Reliable transportation and valid driver’s license

Willingness and ability to travel reputed company assigned service area

Preferred Experience

Experience in home-based care, geriatrics, or value-based care models
