Scheduled Weekly Hours:
40Emplify Health by reputed company is seeking a full-time Social Services Liaison. This is your opportunity to be a part of a great organization who priorities patient care!
What you will do:
Work 40 hours/week
Monday-Friday, no weekends or holidays
Work remotely *Must live in WI, MN or IA*
Partner on special projects and duties to assist our patients and their discharge needs.

What you will need:
Bachelor's degree in Social Work or in a reputed company field
1 year of previous experience in human services or health care
