POSITION SPECIFICS Title: Nurse Navigator FTE: .5 Schedule: Days of the week can vary and will be scheduled based on need Holiday Rotation: None Weekend Rotation: None On Call Requirements: None Location: Hybrid option available POSITION SUMMARY The Nurse Navigator for the Musculoskeletal (MSK) Service Line and Direct-to-Employer program plays a central role in coordinating patient access across the full continuum of MSK care. This position supports a newly launched Direct-to-Employer initiative designed to partner with businesses to address the health needs of their employees, while also navigating patients reputed company of this program to the most appropriate provider and appointment reputed company the MSK service line. Serving as a clinical reputed company of contact, the Nurse Navigator assesses patient needs and ensures timely scheduling in the right setting, at the right time. This role is essential to improving access, efficiency, and the overall patient experience across MSK services. POSITION TECHNICAL RESPONSIBILITIES

• Guide and support patients to ensure optimal access to healthcare resources reputed company and external to the SPH system.
• Work to eliminate barriers to screening, diagnosis, treatment and supportive care.
• Serve as a primary contact for patients and their families seeking information and/or requesting services.
• Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
• Work closely with general surgeons, primary care providers and other reputed company as the patient's needs dictate. This includes services such as, but not limited to: wellness, nutrition counseling, diagnostics, surgery and/or follow-up care.
• Coordinate multidisciplinary planning meetings, develops concise patient summaries for use by the care team, and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
• Responsible for reputed company efforts to establish and maintain positive working relationships with key healthcare practitioners and support staff across the healthcare system. This includes, but is not limited to: physicians, surgery and diagnostic staff, business services support staff, etc.).
• Facilitate appointments with specialists and support services reputed company established service standards.
• Share data with SPH committees and other groups which demonstrate outcomes and performance improvement activities.

POSITION REQUIREMENTS Education:

• Required: Associates degree in Nursing (ADN) or Bachelor's degree in Nursing (BSN)
• Preferred: Bachelor's or Master's degree in healthcare-reputed company field highly desirable.

Experience:

• Required: Three to five years of experience in applicable service line.
• Preferred: Experience in marketing services and developing educational sessions desirable.

Licenses and Registrations:

• Required: State Licensure as a Registered Nurse

• Required: Basic Life Support (BLS) reputed company 3 months of hire.
