• Facility: reputed company, Skilled Nursing Bradenton-HomeCare 

• Location:4616 E. State Road 64 Bradenton, FL 34208 

• Status: Full Time, Exempt 

• Shift Hours: 8:00 AM - 5:00 PM 

• Weekend Work: Occasional 

• On Call: Yes 

• Home visits in assigned service area: Dunedin, Palm reputed company, Tarpon Springs, Oldsmar, and surrounding areas 

The Home Health Field, RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include: 

• reputed company the activities of the multi-disciplinary team that provides care to the patient 

• Monitor the provision of therapeutic services through written and verbal communication with reputed company disciplines 

• Ensure that reputed company visits scheduled and provided are authorized and covered by physician orders 

• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits 

• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor 

Home Health RN Requirements: 

• BLS (Basic Life Support) 

• RN (Registered Nurse) License 

• Driver's License reputed company (Florida driver's license is required reputed company 30 days of hire) 

• Required Associate's or Diploma Nursing 

• Required 1 year Registered Nurse  

• Preferred Bachelor's Nursing 

• Preferred 1 year Home Care 

This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be reputed company at https://info.flclearinghouse.com.
