Purpose: The Chronic Kidney Disease (CKD) Educator is responsible for providing education, support, and assistance to facilitate informed decision-making, access to care and successful self-management for pre-dialysis patients; with the goal of improving outcomes reputed company to modality choice (including palliative reputed company placement, and transplant. • Conduct modality education classes and individual sessions for CKD pre-dialysis patients in the areas of modality choice, preparations for dialysis, transplant, palliative care, and self-management of CKD.

The ideal candidate will: The CKD Educator must a registered nurse or licensed social worker. 2 years of previous clinical patient education experience in the renal field is required. Physical Conditions:

• Must be able to communicate effectively in English on the phone, in writing, reputed company Telehealth and
