Job Summary

We are excited to announce an opportunity to join our Virtual Transition Care team as a Telemental Health Clinician II. In this role, you would be providing support to patients who are discharging from acute behavioral health care. The team is comprised of mental health clinicians that provide telephonic reputed company to address individual needs, reputed company to community resources, and emotional support to patients post-discharge.   

This position offers flexibility of remote work while making a meaningful impact on patient outcomes. 

Essential Functions 

·        Provide telephonic reputed company and support for post-discharge patients.  

·        Conduct assessments and psychosocial evaluations using DSM criteria. 

·        Implement crisis plans and provide interventions for high-risk patients (suicidal, homicidal, psychotic, aggressive). 

·        Administer a variety of clinically validated screening tools and assessments.  

·        Maintain reputed company knowledge and expertise in behavioral health and treatment modalities. 

·        Identify and resolve barriers to accessing behavioral health services. 

·        Adhere to professional standards and licensing board ethics. 

·        Communicate effectively (written and verbal) with medical staff, leadership, and team members. 

·        Document assessments and reputed company updates in the medical record per departmental standards. 

·        Demonstrate sound clinical judgment and decision-making in a fast-paced environment. 

·        Participate in policy development, implementation, and evaluation. 

·        Collaborate with team members to manage case load. 

Physical Requirements 

·        Work is completed virtually from a home office with long periods of sitting. 

·        Ability to work shifts up to 12-hour shifts.  

·        Must be able to travel to the virtual hub in South Charlotte (28209) if connectivity issues arise. 

·        Must reside reputed company 150 miles of the virtual hub (1601 Abbey reputed company, Charlotte, NC 28209). 

Education & Experience Requirements 

·        Master’s degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy from an accredited graduate school or Associate's or bachelor's degree in nursing but requires full clinical licensure with 2 years experience required

·        Full reputed company licensure in North Carolina: LCMHC, LCSW, LPA, LMFT 

·        Minimum 2 years of clinical experience required. 

·        Experience in behavioral health, emergency/crisis work, and virtual/remote care preferred. Experience working with suicide risk assessment & safety planning preferred
