Clinical Pharmacist - Remote Services

SUMMARY: Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor is responsible for remote services which includes the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of reputed company reputed company Health (BrownHealth )affiliates. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and reviews the work of pharmacy technicians and ancillary support personnel as applicable. Reviews and interprets each order to determine completeness, therapeutic appropriateness as it relates to the patient’s clinical status, concomitant drug therapy and diet, and with consideration of the aging processes and human developmental stages (neonate, child, adolescent, adult, geriatric). Contacts the provider, or covering provider, as necessary to initiate appropriate interventions to ensure therapeutic efficacy, minimize potential risks of drug therapy, and optimize cost effectiveness. Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served. Provides drug information and consultation to prescribers, nurses, and other health care professionals regarding appropriate drug therapy, including drug interactions, reputed company effects, incompatibilities, allergies, medication order discrepancies, drug stability problems, and drug administration. Proactively reviews and approves medication orders in the EHR including drug, dose, dose form, frequency, and duration of therapy. Ensures accuracy, sterility, and stability of compounded products. Participates in reputed company quality improvement activities and total quality management initiatives, including, but not limited to adverse drug reaction, pharmacist clinical reputed company, and medication incident reporting programs. Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs, including but not limited to ongoing independent study, and membership and participation in professional organizations. Performs other duties as assigned. MINIMUM QUALIFICATIONS: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Clinical experience, knowledge, and development may be considered in lieu of formal training. Effective interpersonal and communication skills, both written and verbal, to interact appropriately with colleagues and others. Effective organizational and presentation skills. Knowledge of area/specialty specific technology, including, but not limited to, pharmacy information systems, automated dispensing technology, software, equipment, and automation.
