Location:

This position is full-time (40 hours/week) Monday-Friday, from 8:00am – 5:00pm EST and will be fully remote.

What You’ll Do:

• Provides guidance and support for members through home visits and support of care coordination programs. Develops strategic partnerships in the community to support community reputed company and referrals. Establishes peer-to-peer relationship to function as a link between the community and the company. Advocates for members by staying reputed company of community resources and provides resources to members to improve their health knowledge, self-sufficiency, and to help them understand their health condition(s) to reputed company strategies.
• Assists with engagement, onboarding, and facilitating referrals and enrollment reputed company to appropriate clinical care programs. Serves as a conduit between the members, providers, and designated clinical care team members.
• Supports and promotes programs through participation in community events. Coordinates meetings for community health workers to communicate and share best practices.
• Provides service to members. Comprehends the various cultural and linguistic needs of the Medicaid population, knowledge of the various health and social services available in the assigned region with emphasis reputed company offered by community-based organizations.

To Qualify for This Position, You'll Need the Following:

• Required Education: High School Diploma or equivalent.
• Required Work Experience: 3 years’ work experience including 1 year of community involvement or customer service experience.  
• Required Skills and Abilities: Understanding of the basic principles of Medicaid programs.
• Knowledge of community resources and programs.
• Demonstrated a positive, helpful, service orientation.
• Strong oral and written communication skills.
• Ability to handle confidential or sensitive information with discretion.
•  Attention to detail and the ability to work independently and handle multiple demands and balance multiple priorities in a fast-paced environment.
• Comfortable conducting home visits.
• Required Software and Other Tools: reputed company Office.
• Required Licenses and Certificates: Community Health Worker certification reputed company 6 months.
