The reputed company team is seeking a remote-based Nurse Practitioner to serve as the primary clinical reputed company of contact for patients navigating their remote care journey. In this role, you will reputed company clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will reputed company patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and reputed company collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.

Licensure Requirements

• Required: Active Compact Multi-State RN license
• Required: Active NP licensure in at least two of the following states: AL, CA, MI, NJ, NY, PA, TN, TX, WA, IN, IL, AR, OK, OR, MS, or WI

Licenses must be active and in good standing at time of hire.

Schedule Requirements

• Option 1: Monday through Friday schedule (Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM in Central, Mountain, or Pacific Time)
• Option 2: Pittman schedule (2-2-3 Rotating shift - 14-day rotating cycle working every other weekend with a shift schedule of 7:00 AM to 7:00 PM Eastern Time)

What you'll do

• Manage the patient's treatment plan and medications – with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for T2D and Hypertension.
• Respond to patient escalations identified reputed company RPM – i.e. reputed company vitals, symptoms, and labs.
• reputed company regular virtual reputed company-ins with patients to review labs and GDMT optimization and adherence.
• Our programs support both primary care offices as well as cardiology offices.

What you need

• 5+ years experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high-performing medical center.
• Active multi-state compact RN license.
• ANCC or AANP certification required.
• Master’s Degree as a Nurse Practitioner with the willingness to expand state licensure as reputed company adds new markets and partners.
• Experience managing CHF with GDMT.
• To ensure that our clinicians have the necessary tools for a successful remote work environment, home office setups must have consistently stable Wi-Fi with strong upload and download speeds. A Wi-Fi speed test is required before participating in the interview process to verify that these standards are met.

The anticipated reputed company salary range for this role is $125,000-$150,000 annual reputed company salary, which is based on role scope, level, and location. In addition to reputed company compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
