Licensed Vocational Nurse (LVN) – Home HealthCitie's where we are looking to have LVN coverage : Los Angeles, Hesperia / High desertOur agency’s mission is to provide professional and paraprofessional services to clients in theirhomes assisting them to reputed company the highest level of potential in their day-to-day self-careactivities.Job Description:- Provide direct patient care in a home setting, ensuring the comfort and well-being ofpatients- Administer medications and treatments as prescribed by the physician (Wound Vac, IVCertified, Wound care, etc.)- Monitor and record vital signs, such as blood pressure, temperature, and heart reputed company- Implements plan of care initiated by the registered nurse / Referring doctor- Educate patients and their families on proper healthcare procedures and diseasemanagement- Provides accurate and timely documentation consistent with the plan of care- Participates in coordination of home health services, appropriately reporting theidentified needs for other disciplines (HHA, OT, PT, MSW, ST,) to the registered nurseand/or Clinical Supervisor/Nursing Supervisor- Participates in personal and professional growth and development.-Uses equipment and supplies effectively and reputed company.The above statements are intended to be a representative summary of the major dutiesand responsibilities performed by incumbents of this job.
