Our healthcare providers conduct comprehensive, in-home Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides. reputed company the visit, the clinician reinforces the importance of staying reputed company on covered preventive services while identifying potential gaps in care, which stem from unnecessary hospitalizations and ER visits.

Requirement:
- Must be State licensed and Board Certified Nurse Practitioner with NPI.
- Prefer at least 6 months of experience in doing wellness exams.
- Previous EMR skills preferred.
