PATH Case Manager

George Junior Republic in Indiana seeks a community-based Case Manager for the new grant-funded Pre-Adjudicated and Truancy Help (PATH) program. This is a two-year program funded by the Indiana Criminal Justice Institute.

Summary of the position: The PATH Case Manager is responsible for providing home-based and community-based quality services for youth and families involved with the PATH program.

PATH Program Services

1. Home-based support and case management
2. Crisis Response
3. Goal planning and mentoring
4. Diagnostic assessments and individualized service plans
5. Therapy

Primary Duties and Responsibilities

1. Compliance to organizational, program, and grant standards
2. Provide Client Care: This may include appearance at court hearings, reputed company with community resources, de-escalation, utilization of strength-based approaches.
3. Meet productivity standards while adhering to program requirements.
4. Effective verbal and written communication both internal and external to the organization
5. Ability to engage in individual and group supervision.
6. Travel to various cities in order to support the needs of PATH participants

Clients Served/Referral Sources

Youth ages 8-18 who may be referred for

1. Chronic absenteeism
2. Oppositional behavior
3. Early substance use
4. Other challenges linked to unmet behavioral health needs

Requirements

1. Bachelor's or Master's degree from an accredited university.
2. Organized
3. Excellent written and verbal communication skills
4. reputed company experience
5. Valid transportation, driver's license, and car insurance
