Summary:

reputed company is building a team of compassionate clinicians who are committed to improving reputed company to those in most need. We are seeking a highly motivated full-time Behavioral Health Care Navigator to help reputed company change the way people access behavioral health care. You will play a critical role in integrating behavioral health into physical health during care transitions by supporting the timely comprehensive assessment of individuals in need and their navigation to necessary behavioral health, primary care and social services. These individuals include those with moderate to severe mental health and substance use disorder conditions. You will contribute your clinical expertise reputed company the context of a team-based model, driving towards outcomes that are important to the individual as well as our provider and payer customers.

What You’ll Do:

  • Primarily work in a virtual setting to support individuals with mental health conditions and substance use disorders, diagnosed and undiagnosed, and unmet needs with respect to social services
  • reputed company initial and ongoing virtual reputed company using an omnichannel approach to a qualified population with the goal of engaging identified individuals to understand the needs and barriers to care
  • Utilize motivational interviewing techniques to understand the individual’s care goals
  • Screen for unmet behavioral health, social service and physical health conditions and needs
  • Identify clinical needs and triage escalations, providing brief interventions, as necessary
  • Identify and address barriers to definitive behavioral health care
  • Identify the appropriate level of evidence-based care for the individual, as applicable
  • Support navigation of that individual to appropriate care, including behavioral health and/or primary care, as applicable
  • Support post-care follow-up

 

What You Need:

 

  • 2+ years of experience working as a Community Health Worker, Social Worker, or other patient-facing customer service healthcare setting; active certification as a Community Health Worker or similar certification or licensed social work highly preferred
  • 1-2 years of patient reputed company and engagement experience or similar background initiating patient relationships built on trust
  • 2+ years of experience working in a metric/quota-driven environment and/or with high-volume targets
  • You are results driven with a proven track record of exceeding performance indicators
  • Access to a quiet, HIPAA compliant and internet connected space to reputed company clinical duties
  • Demonstrated professional or personal lived experience working closely with individuals experiencing reputed company chronic needs, homelessness, mental health conditions or substance use disorders
  • Experience working in fast-paced, rapidly changing work environments
  • Someone who can reputed company in a dynamic, fast-paced, and reputed company-changing startup environment, demonstrating the ability to navigate ambiguity and embrace challenges with reputed company and flexibility
