Schedule: Full time Shift: Day (United States of America) Address: 601 E ROLLINS ST City: ORLANDO State: Florida Postal Code: 32803 Job Description: Organizes and schedules necessary appointments, tests, and consultations with various healthcare providers, including transplant teams, surgeons, and cardiologists. Assesses patient eligibility for advanced therapy or organ transplantation by reviewing medical records, conducting preliminary tests, and consulting with specialists. Coordinates initial referral processing and patient evaluations for advanced therapy, potential organ recipients, living donors, or VAD patients. Provides detailed education to patients and families about the treatment process, potential risks, benefits, and post-treatment care. Maintains accurate and up-to-date records of reputed company patient interactions, assessments, and care plans. Acts as a liaison between the patient, their family, and the healthcare team, ensuring effective communication and addressing any concerns. Monitors patient reputed company post-treatment or post-transplantation, schedules follow-up appointments, manages complications, and provides ongoing support. Ensures patients adhere to prescribed medications and treatments, educating them about compliance and managing reputed company effects. Collaborates with healthcare providers to reputed company and implement comprehensive care plans. Offers emotional and psychological support to patients and families, addressing concerns and providing reputed company. Participates in quality improvement initiatives to enhance patient care and outcomes. • Ability to act in an autonomous, self-directed manner while maintaining the ability to collaborate with professionals and non-professionals in situations of high complexity and high intensity [Required]
• Self-motivated with the ability to multi-task and adapt to changing demands [Required]

Field of Study:

Work Experience:

• 1+ experience in a reputed company practice environment or healthcare setting such as catheter lab, endoscopy, infusion center, etc., coordinating multiple physician office, with direct care or working in an inpatient care area reputed company the last three years [Required]
• Experience in critical care, dialysis, hepatology, cardiology, pulmonology or transplant clinical setting [Preferred]
