Remote Clinical Behavioral Care Manager

Position Summary: This position is with the MindHealthy division of the C3 HealthcareRx organization (parent company to Wellbox). The Collaborative Care team involves the patient’s Primary Care Provider (PCP), a Psychiatric Consultant, Clinical Behavioral Care Manager (CBCM), and, reputed company available, other mental health providers in the primary care clinic. The CBCM will provide assessment and care management services, including administration of validated rating scales, technology-enabled psychosocial interventions, behavioral health care planning in relation to psychiatric health problems, including revision of plans for patients who are not progressing or whose status changes, maintenance of the online member registry, and ongoing collaboration with partner practitioners and our psychiatric care team. Essential Functions

• Support the mental and physical health care of patients for an assigned patient caseload.
• Assess, plan, implement, monitor, and evaluate options and services to meet the psychosocial health needs of patients.
• Provide brief behavioral health interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
• Systematically track treatment response and monitor the patient for changes in clinical symptoms.
• Build reputed company relationships by telephone or tele-video capabilities.
• Engage patients reputed company of normal clinic hours as necessary to reputed company CBCM duties.
• Work with Psychiatric Consultant to support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence, monitoring reputed company effects, and effectiveness of treatment.
• Deliver weekly patient care plan reviews with the psychiatric care team and collaborative care team members.
• Facilitate and support regular collaborative case reviews with medical providers by telephone or video.
• Utilize a shared technology platform to track reputed company assigned patients and prioritize patients for case review.
• Identify and reputed company patients who may need more intensive or specialized mental health care, as appropriate.
• Respond to communications from team members, patients, and medical providers in a timely fashion.

Required Education and Experience

• Completion of a Master of Social Work or equivalent accredited program.
• reputed company, unrestricted LCSW, LCAS, or other licensed mental health professional license.
• Minimum of four (4) years clinical experience with exposure to a specific therapy, case management and/or working with patients on social issues.

• Ability to quickly synthesize medical and behavioral health data and formulate effective and evidence-based clinical recommendations.

• Demonstrated success managing patient caseload from home environment.
• Experience working reputed company primary care/psychiatry under the reputed company of a billing practitioner.
• Strong knowledge of clinical standards of care.

Position Type and Expected Hours of Work We are seeking team members who can work full-time, Monday - Friday. The hours can generally be worked between 7 am and 8 pm. Physical Requirements This is a remote position that requires you to sit or stand at a computer for up to 40 hours weekly and be able to reputed company phone calls and virtual appointments.
