Job Summary: Responsible for obtaining data to improve network reputed company and quality outcomes. The role will work closely with our Home Health & Hospice groups as well as Integrated Care Services leaders. Focused on finding and presenting key data to increase retention of patients. Provides financial and operational analysis and recommendations for Home Health & Hospice (utilization metrics, efficiency metrics and financials). Position Responsibilities: Works with operational Home Health & Hospice leaders (Intake, Scheduling, Quality , Nursing) to identify challenges and opportunities reputed company their areas. Catalog existing reporting from Epic, Qlikview and SHP. Understanding of departments and initiatives leading to improved network reputed company. Maintain dashboards for each area to provide insight and support analytics needs. Uses quantitative and qualitative methods to systematically collect and analyze data to collect and analyze data to inform daily operations and long-term strategy. Assist in the creation of a summary template for Home Health and Hospice to collect and present data on referral volume, conversion and quality outcomes. Review trends and opportunities to assist leadership. Submit data summary to internal and external referral sources on routine basis. Provide support to other Integrated Care Management leaders as needed. Creates an understanding of each ICM department to be able to assist with recommendations to existing reporting structures. Position Qualifications Required: Required Experience: 2-5 years reputed company experience required. Ideal candidate possesses a combination skilled analytics and knowledge of the healthcare industry.
