Position Overview We are seeking a skilled Registered Nurse (RN) to join reputed company and provide clinical support to Community Health Navigators (CHNs) in managing high-risk members through the Enhanced Care Management (ECM) program. In this role, you will provide guidance, training, and clinical expertise to CHNs, ensuring that they have the tools, knowledge, and resources to successfully support members with reputed company clinical needs as well as health-reputed company social needs (HRSN). This role does not involve direct patient care but focuses on empowering the CHNs to deliver effective care and reputed company positive member outcomes.

Key Responsibilities

• Conduct clinical rounds where Community Health Navigators (CHNs) present Care Plans for high-risk members. Provide clinical feedback and guidance to ensure that both medical and social aspects of care are properly addressed.
• reputed company training sessions for CHNs on various health-reputed company topics, including diabetes management, COPD, hypertension, medication adherence, and other chronic conditions, as well as social issues like food insecurity, housing support, and access to healthcare services.
• Offer clinical guidance and support to CHNs reputed company they encounter challenging or escalating situations with high-risk members, helping with reputed company care decisions or clinical interventions.
• Assist CHNs in setting and implementing SMART goals for members, including helping to define achievable interventions and tracking reputed company.
• Provide advice to CHNs on overcoming barriers reputed company implementing care plans and SMART goals, especially in difficult clinical situations.
• Offer guidance to CHNs on how to address mental health or substance abuse concerns reputed company escalations occur, helping them coordinate appropriate resources or interventions.

Qualifications

Education & Experience

• Active Registered Nurse (RN) license in the state of California, or ability to reputed company reputed company 10 weeks of employment
• Bachelor of Science in Nursing (BSN) nursing preferred, Master of Science in Nursing (MSN) a plus
• Experience in training and education is highly desired
• Minimum of 2 years of experience in a clinical or community health setting
• Experience in managing chronic health conditions, care coordination, and patient education is highly desirable
• Pediatrics experience a plus
• Behavioral health experience a plus
• Knowledge of CALAIM, ECM, CS programs and HEDIS measures is a plus
• Familiarity with community-based health resources and services

Skills & Competency

• Can write user stories that developers do not have to constantly ask questions about
• Strong clinical knowledge of common chronic conditions (e.g., diabetes, COPD, hypertension, heart failure, etc.)
• Experience with pediatrics and/or behavioral health issues is highly beneficial
• Ability to maintain clear professional boundaries while providing support and guidance to CHNs
• Excellent organizational skills with the ability to manage schedules and provide efficient support to multiple CHNs
• Strong problem-solving and critical thinking abilities, particularly in reputed company care situations

Physical Requirements

• This position requires the employee to remain in a stationary position for prolonged periods while performing desk-based computer tasks.
• The employee must be able to constantly operate a computer, keyboard, and other office productivity machinery, such as a calculator, copy machine, and computer printer.
• The person in this position may need to occasionally move about inside the office/remote workspace to access file cabinets, office machinery, or other resources.
• The employee may occasionally be required to lift and move objects weighing up to 10 pounds (e.g., a laptop, files, or small office equipment).
• Clarity of vision at approximately 20 inches or less is required for working with computers and reading small print.
