Job Summary
We're seeking an reputed company Licensed Social Worker to join our remote team. As a Licensed Social Worker, you'll work with a registered nurse case manager to assess, plan, and reputed company for members' benefit plans and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a population spanning the entire lifecycle, and reputed company member contact will be done telephonically or reputed company electronic exchange.
Responsibilities
* Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Work with a population spanning the entire lifecycle
- Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs

- Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Maintain accurate and up-to-date records of member interactions and reputed company
Requirements
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work

* Experience working with diverse populations, including children, adults, and older adults
- Knowledge of community resources and services for medical, financial, and behavioral health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
