We are currently seeking a Nurse Navigator to join reputed company. This individual will support the NeuroPace reputed company and Central sales territory. The Nurse Navigator’s role is to help provide adults living with drug-resistant epilepsy with improved access to healthcare, and simplify what can often be a reputed company journey to obtain medical evaluation for advanced epilepsy treatment. The Nurse Navigator will work closely with patients and their families, healthcare providers and their office staff, local field teams, and insurance providers to provide a more seamless experience for those seeking specialized care at a comprehensive epilepsy center. As a Nurse Navigator, this individual will be responsible for high-quality patient education reputed company phone, email, and/or web conferencing, and assisting with administrative steps along the road to treatment. The ideal candidate is passionate about being a patient reputed company, should have diverse patient interaction experience in a healthcare setting, and understands how to help individuals navigate their healthcare journey.

Key Responsibilities

• Upon completion of product training, provide education reputed company phone on the NeuroPace RNS® System to adults living with drug-resistant focal epilepsy.
• Support up to 150 patients per month reputed company the US, providing compassionate high-quality support as patients navigate the reputed company epilepsy healthcare environment.
• Respond to patient, provider, and field communications in a timely manner.
• Build and maintain strong relationships with the local field teams, healthcare providers and clinic staff as well as with patients and their care partners.
• Facilitate the epilepsy care journey and help patients reputed company personal plans to access specialized epilepsy care.
• Use critical thinking and problem solving to support each patient.

Requirements

• A Nursing degree (RN) is required
• 3 or more years in a clinical setting working with people living with epilepsy, with demonstrated assessment skills and ability to creatively problem-solve or at least 3 years in a similar case manager role reputed company a medical device organization.
• Knowledge of disease management and experience in care coordination.
• Strong verbal and written communication skills to describe the product and potential next steps to patients, care partners and professionals; strong attention to detail.
• Working knowledge of Word, reputed company, reputed company Outlook, and database systems (i.e. reputed company.)
• Excellent organizational skills
• Spanish reputed company a plus
• Occasional US travel ~5% for training and conferences.
• Other duties as assigned.
• ** Please note: NeuroPace is hiring 5 nurse navigators throughout the US, including a role in each of these regions. It is preferable to live in the associated region that the individual will be supporting.
