Assists other members of the health care team in providing the highest quality personalized patient care. Maintains a positive work relationship with the health care team. Education Required - Graduate of an accredited school of practical nursing. Work Experience Required - 2 years of ambulatory care experience. Job Duties

• Oversees the disease registry database.
• Conducts pre-visit chart review of patients.
• Coordinates care across the continuum.
• Participates in quality improvement activities.
