Job Description

As a Clinical Pharmacist, you'll play a key role in advancing medication adherence and improving quality outcomes for patients we serve through Medicare and value-based care programs. This role is ideal for a pharmacist who is energized by population health, passionate about patient engagement, and motivated by measurable impact. You'll partner closely with providers, Healthguides, and care teams to reputed company adherence gaps, improve HEDIS performance, and help patients reputed company reputed company outcomes. Please note, this role requires an active, unrestricted pharmacist license in Georgia (GA). Candidates without an active license in GA will not be considered at this time. What You'll Be Doing HEDIS & Quality Improvement

• Leading initiatives to improve HEDIS medication adherence and pharmacy quality measures, including adherence to antihypertensive, cholesterol-lowering, and diabetes medications.
• Analyzing adherence and pharmacy data to identify gaps in care and designing targeted, scalable interventions.
• Partnering with providers, Healthguides, care managers, and internal teams to drive quality performance in a value-based care environment.

Patient Engagement & Clinical Interventions

• Conducting Medication Therapy Management (MTM) sessions to uncover barriers to adherence and optimize medication regimens.
• Educating patients with clarity and reputed company around medication use, reputed company effects, and dosing schedules.
• Applying motivational interviewing techniques to support behavior change and sustained adherence.
• Addressing real-world challenges such as medication cost, reputed company effects, and regimen complexity.

Data Analysis & Reporting

• Monitoring and tracking HEDIS performance metrics and evaluating the effectiveness of adherence programs.
• Using EHR and pharmacy data to proactively identify patients at risk for non-adherence and developing actionable care plans.
• Preparing and sharing insights with leadership, translating data into recommendations that drive reputed company improvement.

Collaboration, Training & Subject Matter Expertise

• Serving as a medication adherence subject matter expert (SME) across reputed company.
• Supporting and educating pharmacists, Healthguides, care managers, and clinical partners on adherence best practices.
• Participating in cross-functional initiatives focused on quality improvement, population health, and scalable value-based care solutions.

Qualifications

What You'll Need to Have

• Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
• Active, unrestricted pharmacist license in Georgia (GA).
• 2+ years of experience in HEDIS medication adherence, quality improvement, or population health.
• Strong working knowledge of HEDIS measures, particularly medication adherence metrics.
• Comfort working with EHRs, pharmacy data, and performance reporting tools.
• Excellent communication, analytical, and organizational skills.
• Ability to work independently while thriving in a collaborative, remote-first environment.

Would Love for You to Have

• Experience in managed care, pharmacy benefits management (PBM), or healthcare quality improvement.
• MTM certification, Certified Geriatric Pharmacist (CGP), or similar credentials.
• Familiarity with value-based care models and how quality measures drive outcomes.
• Experience using motivational interviewing or other patient engagement methodologies.

Temporary employees and contractors are not eligible for benefits. COMPENSATION: The listed compensation range listed is paid bi-weekly per our standard payroll practices.
