Bilingual Spanish Care Manager (reputed company reputed company) locations Long Beach Office 3800 Los Angeles, CA time type Full time posted on Posted 2 Days Ago job requisition id JR2223 The Job Provide long-term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers in order to help them remain safely at home and to prevent premature institutionalization. You Will Conduct in home and telephonic assessments, care planning, and client monitoring through contacts with the client, professionals and client’s support system. Coordinate linkages with community-based services and resources. Maintain legal and clinical written record of each client’s case by completing reputed company necessary documentation (including assessment, care plans, service arrangements and client monitoring reputed company notes) in a timely manner. Participate regularly in Case Conference, Quality Assurance chart reviews, completing other audits as required, helping to train new employees, participating in training sessions, and responding positively to suggestions for improvement. Other duties as assigned. Your Qualifications Bachelor's Degree in Social Work, Psychology, Gerontology or reputed company field required. Master's Degree in Social Work (MSW) preferred. BILINGUAL- Must be bilingual in English/Spanish. (Test will be administered to assess proficiency.) 1-2 years experience working with the elderly. 1+ year experience working in a hospital, home health agency or reputed company setting in which the primary targeted population is older adults. Ability to travel reputed company program’s catchment area and visits to client homes is required.
