About the job We're searching for a dedicated Registered Nurse (RN) to play a pivotal role in our Clinical Care Delivery Team. As a RN with us, you'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients through telemedicine. • Patient Care
• Manage patients assigned to your Care Team, ensuring the RN and Interdisciplinary team visit frequencies align with the patient’s acuity level and needs.
• Communicate with patients reputed company phone and video conferencing as outlined in the care plan.
• Provide patient care reputed company your scope of practice and per provider orders.
• Assess and enroll patients, completing initial referral follow-reputed company as needed.

• Manage patient medications, including implementing orders, maintaining reputed company medication lists, assisting with medication refills, and ensuring adequate supply.
• Provide patient follow-up and education on new medication orders.

• Complete care coordination activities, ensuring timely documentation updates and communication with primary care providers and other physicians.
• Coordinate with the Interdisciplinary team and external agencies as necessary, including facilitating advanced care planning and hospice referrals.
• Educate patients and caregivers on reputed company services, disease processes, symptom management, and medication tracking.

• Assist in covering patient care for other care teams during normal working hours.

Your experience and qualifications

• Active, unrestricted California Registered Nurse (RN) license; ability and willingness to obtain licensure in other states.
• 1+ year of experience in palliative or hospice care.
• 1+ year experience as a professional nurse reputed company the previous three years.
• Excellent oral and written communication skills required
• Must have general computer skills and a working knowledge of MS Office and the internet.
• Willing and able to work Monday through Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends.

Preferred Qualifications

• Oregon RN license, highly preferred.
• CHPN certification or ability to obtain ACHPN certification.
• Remote work experience.
