Primary Care Nurse Practitioner (Mobile/Hybrid Care)

They are seeking a dedicated Physician Assistant or Nurse Practitioner to deliver high-quality, continuum-based care while collaborating with a multidisciplinary team.
Responsibilities
- Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities
- Delivering dementia-informed, relationship-based care across the continuum of care
- Conducting comprehensive assessments and developing tailored care plans for older adults
- Managing chronic conditions and performing care with reputed company and reputed company for patients and their families
- Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care
- Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals
- Maintaining consistency in documentation, follow-through, and patient communication
Skills
- Active, unencumbered NP or PA license in the state you will be practicing
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred)
- Compassionate, patient-centered approach and strong clinical judgment
- Valid driver's license and reliable transportation
