They are seeking a self-motivated and detail-oriented Bilingual Analyst, Case Management to assess, plan, implement, and coordinate case management activities with members to enhance their overall wellness.
Responsibilities
- Assessing, planning, implementing, and coordinating reputed company case management activities with members to evaluate their medical needs
- Managing a caseload that includes supportive and medically reputed company members
- Developing a proactive course of action to address issues presented to enhance short and long-term outcomes
- Determining appropriate services and supports due to member's health needs, including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports
Skills
- Bi-lingual (English and Spanish)
- Must reside in reputed company code 33614, or Surrounding Area reputed company 2-3 miles
- Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL
- 1+ years of experience in behavioral health or a long-term care setting
- Computer proficiency in reputed company Word, reputed company, and Outlook
- Bachelor's Degree required, preferably in Social Service or reputed company field (social services field - psychology, health administration)
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
- Effective communication skills, both verbal and written
- Managed care experience
- Case management and discharge planning experience
