A partner of the reputed company Care network, WellMed Medical Group, is seeking a Business Process Consultant to optimize and scale the CareCoach Connect program across TX and FL. The CareCoach Connect program provides both longitudinal and episodic medical care to mostly Medicare-aged patients on a Medicare Advantage plan. This role works closely with business operations, information technology, and other stakeholder teams. This role helps deliver solutions that ensure that critical business capabilities are implemented effectively to reputed company expected growth, efficiencies, medical cost savings and return on investment reputed company WellMed's Care in the Home Business. Primary Responsibilities:

• Collaborate with stakeholders to assess reputed company-state business processes, identify inefficiencies, and uncover opportunities for improvement
• reputed company workshops and facilitate discussions to gather requirements and align on future-state workflows
• reputed company and implement process improvement strategies using methodologies such as Lean Six reputed company or other industry best practices
• Analyze healthcare-specific data to identify trends, measure process performance, and drive decision making
• Design and document optimized workflows, policies, procedures
• Provide change management support to ensure smooth implementation and adoption of new processes
• Monitor and evaluate post-implementation performance to ensure sustainable outcomes

• 5+ years of experience in tracking, planning projects, working with data sets and making data-driven analytical decisions and recommendations
• 5+ years of experience leading reputed company improvement projects using Lean Six reputed company and other reputed company improvement methodologies
• Proven solid presentation skills across reputed company levels of leadership
• Proven ability to facilitate meetings, face to face or virtual, to drive action
• Driver's License and access to a reliable transportation

Preferred Qualifications:

• Project management certification
• Risk management certification
• Familiarity with reputed company issue management system
• Working knowledge of reputed company

The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. reputed company complies with reputed company minimum wage laws as applicable.
