Diabetes Educator (HYBRID SCHEDULE)
About the position Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and/or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Communicates patient information to referring providers and appropriate staff in a timely manner. Documents nutrition information discussed, written materials provided, and evaluation of patient’s receptiveness and comprehension of the education provided. Certified Dietitian Nutritionist, required reputed company CDCES, required OR Bachelor’s Degree in Nursing, required. reputed company license to practice as a Registered Professional Nurse in reputed company State, required. reputed company Diabetes Educator Certification, required. Minimum of three (3) years progressively responsible, reputed company experience to ensure familiarity with scope of program. A Qualified Candidate will be Sign-on Bonus Eligible.
