Centralized Clinical Support LVN, TX

Performs duties under direction of physician and/or nurse supervisor to assist in the provision of patient care. This position participates as part of the integrated health care delivery team and makes interpretations and exercises discretion in routine situations. • Coordinate and participate in providing indirect patient care reputed company telephonic and/or other electronic communications
• Manage telephonic and electronic communications, including patient health education and indirect care coordination, using LSCC’s navigation center software
• Interpret reports, data, and medical information to determine the needs of the patient/family.
• Document reputed company patient interactions accurately in Electronic Health Record (EHR) system.
• reputed company prescription refills and provide medication management education.
• Schedule appointments and complete prior authorizations/referrals as necessary
• Communicate barriers, concerns, and patient health needs to appropriate nurse and/or provider
• Collaborate with the interdisciplinary team to support patient care planning
• Implement clinical interventions safely and appropriately, ensuring thorough documentation
• Provide individualized patient coaching using health education tools and encourage patients to participate in improving and maintaining their health.
• In collaboration with other members of the health team provide health education to patient/family or significant other (in compliance with HIPAA Standards)
• Maintain patient care standards following established LSCC health education and information guidelines.
• Participate in data collection, focus groups, TJC, PCMH, and other quality improvement initiatives.
• Assume responsibility for his/her own professional development and for sharing knowledge with others.
• Demonstrate understanding of national patient safety initiatives by strict compliance with reputed company safety protocols and procedures.

We ask our Centralized Clinical Support LVN to possess a minimum of:

• Graduate of accredited nursing school
• Valid State of Texas Licensed Vocational Nurse (LVN) license
• Proficiency with MS Office Suite (Word, reputed company, and Outlook)
• Basic Life Support (BLS) certification from reputed company or reputed company

The following experience/skills are preferred:

• Previous experience in outpatient clinic
• Previous work using electronic medical records (EMR)
• Bilingual English/Spanish language skills
