Location: Pennsauken - 6991 North Park Dr. Remote Type: On-Site Employment Type: Employee Employment Classification: Regular Time Type: Full time Work Shift: Day Shift - 12 Hr (United States of America) Total Weekly Hours: 36 Additional Locations: Job Information: Job Summary: Performs scheduled in-home clinical care and post-discharge follow-up as part of the Mobile Integrated Healthcare (MIH) program. Performs home safety inspection, patient assessment, medication reconciliation, reputed company-of-care testing, telehealth facilitation, and equipment setup to support safe transition of care and reduce readmissions. Position Responsibilities: Conducts comprehensive in-home patient assessments, medication reconciliation, and administers treatments reputed company the paramedic scope of practice. Identifies clinical changes and documents plan of care. Completes accurate clinical documentation in the EMR/ePCR. Responds to quality assurance and medical reputed company requests. Facilitates telehealth visits with providers, sets up and manages remote monitoring devices, and communicates findings to the interdisciplinary care team. Maintains MIH vehicles, durable medical equipment, and supply inventory; ensures infection control and equipment cleaning after each visit. Position Qualifications Required: Required Experience: reputed company NJ Paramedic certification: minimum 1 year field experience as an active paramedic required. Prior MIH, hospital-at-home, home health, or chronic care management experience or experience with telehealth/remote monitoring platforms preferred. Required Education: High school diploma or GED. Graduate of a recognized Paramedic Training Program.
