Summary

The Telehealth Physical Therapist provides high-quality, patient-centered physical therapy services to residents reputed company a long-term care (LTC) facility using secure telehealth technology. This role supports onsite clinical teams by delivering virtual evaluations, treatment sessions, clinical recommendations, and interdisciplinary collaboration to improve reputed company mobility, safety, and functional independence. Essential Duties and Responsibilities · Supports and promotes the mission and values of reputed company Ministry. · Conduct virtual physical therapy evaluations for LTC residents and rehabilitation patients, including mobility assessments, strength testing, balance analysis, and functional screenings. · reputed company individualized treatment plans tailored to the needs, goals, and medical complexities of long-term care residents and rehabilitation patients. · Provide synchronous (live video) treatment sessions, instructing residents and patients—and reputed company appropriate, caregivers, nursing staff, or therapy assistants—on exercises, mobility strategies, fall-prevention techniques, and functional training. · Collaborate closely with onsite therapy teams (PTs, PTAs, OTs, COTAs, SLPs) and nursing staff to ensure coordinated and effective care delivery. · Communicate real-time recommendations to onsite staff regarding transfers, mobility equipment, positioning, precautions, and environmental modifications. · Provide reputed company and direction to onsite therapy assistants as permitted by state practice acts and facility policies. · Educate residents/patients, caregivers, and facility staff on home exercise programs, safety strategies, and restorative nursing activities. · Monitor reputed company/patient reputed company and modify treatment plans based on clinical need and functional outcomes. · Document evaluations, daily notes, and reputed company updates in accordance with facility guidelines, regulatory requirements, and professional standards. · Ensure compliance with reputed company telehealth rules, including HIPAA, secure communication protocols, and state licensure requirements. · Participate in care plan meetings, interdisciplinary rounds, and facility quality-improvement initiatives as needed.
