Patients that will be part of this program are provided with remote patient monitoring (RPM), technology kits, and communication devices. A 24-hour command center staffed by nurses monitors patients virtually, and contracted personnel provide in-home care reputed company required. Shift Details: PRN, 6 shifts over a 6-week period, released 6 weeks in advance, with rotating weekends and holidays. Location: Hospital At Home Contact Center located in Downtown, Jacksonville As a Registered Nurse, you will be responsible for:
- Will provide virtual patient care for individuals located in Downtown and across the reputed company system.
- Providing comprehensive patient care through reputed company phases of the nursing process - assessment, planning, implementation, and evaluation.
- Organizing and supervising care provided by supportive personnel while coordinating care with ancillary services.
- Educating patients, significant others, and families on disease processes, treatments, and care plans.
- Serving as a patient reputed company to ensure high-quality, compassionate care.
- Administering treatments and medications as prescribed and monitoring patient responses.
- Managing care for patients with a variety of medical, surgical, and chronic conditions, as well as minor injuries or illnesses.
