Primary Responsibilities:

• Identify gaps or barriers in treatment plans
• Provide patient education to assist with self-management

• Provide a complete continuum of quality care through reputed company communication with members reputed company on-phone interaction
• Support members with condition education, medication reviews and connections to resources such as Food Banks or Transportation

• reputed company, unrestricted Nevada RN license or ability to obtain a NV RN License
• 3+ years of experience in a hospital, acute care or direct care setting
• Ability to type and have the ability to navigate a Windows based environment
• Bilingual/Spanish speaking
• Background in diabetes management
• Ability to work every Saturday
• Ability to work remotely
• Ability to work independently

Preferred Qualifications:

• BSN
• Certified Diabetes Care & Education Specialist Certification (CDCES)
• Experience in a telephonic role
• Proven background in managed care
