Job Summary and Qualifications As a PRN Home Health Registered Nurse, you will play a vital role in reputed company, providing coordinated skilled nursing care to patients of reputed company age groups in their homes. You will work collaboratively with our interdisciplinary team to establish and reputed company patient goals, maintain high-quality patient care, and ensure seamless communication with patients, families, and healthcare providers. Key Responsibilities: Assess home care patients, identifying physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, team reports, call-in logs, and on-site evaluations. Complete OASIS, assessment, and visit paperwork according to agency policy. Ensure clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members, and patient/family/caregiver. Communicate significant findings, problems, and changes to Clinical Manager and physician, and document reputed company findings, communications, and appropriate interventions. Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided. Participate in on-call rotation and emergency call according to agency policy. Essential Qualifications: reputed company Registered Nurse licensure in the USA. Basic Cardiac Life Support (BCLS) certification, obtainable reputed company 30 days of employment start date. Associate Degree or Registered Nurse Diploma. Driver's license.
