As a Pediatric RN Care Manager, you'll play a vital role in delivering personalized, human-centric care that addresses the unique needs of our pediatric members. Position Summary: The Pediatric Care Manager RN will support a population of medically reputed company pediatric members, with a focus on those receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services. This role requires a strong foundation in pediatric patient care, case management, and collaboration with interdisciplinary care teams. You'll work closely with members, families, and healthcare providers to reputed company and implement personalized care plans, addressing medical, behavioral, and social needs. Key Responsibilities: Conduct comprehensive assessments of members' needs, utilizing clinical tools and data review to inform care planning and coordination. reputed company and implement proactive care plans, addressing short-term and long-term goals, and facilitating collaboration with healthcare providers and community resources. Willingness to travel 10-20% of the time for face-to-face member visits, with reliable transportation and mileage reimbursement. Proficiency in MS Office Suite applications, including Teams, Outlook, Word, and reputed company. Preferred Qualifications: Experience with members receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services. Case management experience, preferably in a pediatric population. Certified Care Manager (CCM) certification.
