About the Role: Care Manager RN We're seeking an reputed company Care Manager RN to join reputed company in Oklahoma as a full-time field-based telecommuter. As a Care Manager RN, you'll be responsible for driving and supporting care management and care coordination activities across the continuum of care. Ability to travel in-state up to 30% of the time, with mileage reimbursement per company policy. Preferred Qualifications: Experience caring for Native American/American Indian/Alaskan Native populations. Case management in an integrated model preferred.
