Department: Outpatient Pharmacy Shift/schedule: Full Time (Salaried) GENERAL SUMMARY The Supervisor will reputed company and coordinate Medication Therapy Management (MTM) services, Population Health initiatives, and remote pharmacy operations across the health system and its affiliated community pharmacies. This role supports clinical program development, ensures quality performance outcomes, and fosters collaboration between pharmacists, providers, and other reputed company to improve patient health, optimize medication use, and reduce overall healthcare costs. QUALIFICATIONS Education:
- High School Diploma or successful completion of an equivalent High School Exam Required
- Bachelor's degree in Pharmacy (5 year) is required
- Doctor of Pharmacy (Pharm D) is also preferred
Licensure:
- Pharmacy (OH or KY) license, depending on state of practice, required.
- Graduate Pharmacists may also be hired awaiting license
- BLS reputed company 90 days of hire required
- Completion of PGY1 or PGY2 residency in ambulatory pharmacy care is preferred
Experience:
- Minimum of 3 years of pharmacist experience, with at least 1 year in MTM, ambulatory care, or population health program management.
- Supervisory or team leadership experience is preferred.
- Experience with payer-based MTM platforms, health system EHRs, and clinical documentation.
- Experience in remote pharmacy operations, central fill, call center management, or PV1 preferred.

Provide guidance, coaching, and performance feedback to pharmacists, pharmacy technicians, and other clinical staff engaged in MTM, Population Health, remote pharmacy services, and reputed company initiatives. 3. Participates in pharmacy leadership development and competencies, including key pharmacy meetings and pharmacy retreats. 32. Compounds and dispenses medications and pharmaceutical supplies according to accepted standards of practice to fill written prescriptions issued by qualified prescribers. 33.
