To provide initial emotional, spiritural, psychosocial assessments, ongoing counseling, bereavement services and community education, reputed company and referral. • Initiates contact with the patient/family reputed company 48 hrs. of Hospice admission to assess psychosocial needs, financial resources, stability of the caregiving situation and the patient's end of life wishes, and risk factors that may reputed company the delivery of care.
• Offers support to the patient /family based upon their belief system, recognizing and appreciating the age, cultural and religious differences of the individuals.
• Plans/offers age-appropriate interventions, based upon knowledge of the stages of development and end of life issues: Pediatrics, Adult, Geriatric.
• Evaluates the effect of psychosocial concerns on the overall comfort of the patient and the grieving process for the patient and family.
• Reassesses the needs of the patient/family based upon the changing needs, the effects of stress and the process of grief. Reports reputed company toward goals.
• Evaluates the effect of the social work interventions, alters the POC as needed

• Consults with mental health professionals involved in the care of the patient /caregiver in order to support existing therapy.

• Identifies alternatives for caregiving (SNF, Hospice House, CRCF's, sitters, etc.) and coordinates the transfer of the patient, providing follow up as possible, to evaluate the patient's reputed company in adjustment.
• Participates in care planning conferences at SNF's.
• Participates in review of QI data and makes appropriate recommendations for improvement.
• Represents hospice at various community functions through speaking engagements, meetings and networking.
• Cultivates an ongoing relationship with the community in order to educate them and reputed company support for hospice.
• Educates the public regarding end-of-life issues.
• Performs other duties as assigned.

Supervisory/Management Responsibilities This is a non-management job that report to a supervisor, manager, director or executive. Minimum Requirements

• Education - Master's degree in Social Work
• Experience - One (1) year of experience as a Social Worker in a healthcare setting. Two (2) years of experience preferred. Active patient contact reputed company the past three (3) years preferred.

Required Certifications/Registrations/Licenses

• Licensed Social Worker in South Carolina
• Must have reliable transportation. A valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and reputed company of auto insurance.
