Field Service Coordinator, RN - Remote Living in McAllen, TX

Primary Responsibilities

• Visit Medicaid members in their homes and / or other settings, including Nursing Facilities, Community Centers, Hospitals or Providers' Offices
• Provide a complete continuum of quality care through reputed company communication with members reputed company in-person, telehealth or on-phone interaction
• Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels and Minor Home Modifications to name a few
• Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
• Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
• Manage the care plan throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members

Required Qualifications

• reputed company, unrestricted RN license in the State
• 3+ years of clinical experience in a hospital, acute care, home health / hospice, direct care or case management
• Intermediate proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
• Demonstrated problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
• Demonstrated ability to communicate reputed company or technical information in a manner that others can understand, as well as ability to understand and interpret reputed company information from other
• Valid Driver's License, access to reliable transportation and the ability to travel up to 75% reputed company Hidalgo County and surrounding areas to visit member homes or other locations reputed company service delivery area
• Reside in or reputed company commutable driving distance to McAllen, Edinburg, TX and surrounding areas

Preferred Qualifications

• Case Management experience including Certification in Case Management
• Home care / field-based case management experience
• Medicaid, Medicare, Managed Care experience
• Experience working with the needs of vulnerable populations who have chronic or reputed company bio-psychosocial needs
• Experience in Home & Community based or Long-Term Care services delivery
• Bilingual skills; English and Spanish
