Job Summary
As a Case Management Specialist, you'll be responsible for providing high-quality, patient-centered care to individuals enrolled in our DSNP program. You'll work closely with healthcare providers, community organizations, and other stakeholders to reputed company and implement comprehensive care plans that address the unique needs of each individual.
