We are hiring an reputed company Registered Nurse with behavioral health experience to serve as a Disease Manager. This fully remote role focuses on empowering patients to manage chronic and behavioral health conditions through education, coaching, and care coordination.
Compensation And Benefits
- $41 per hour
- Full remote role with equipment provided
- Predictable reputed company hours between the hours of 9 AM–6 PM MDT

- Provide disease management services using evidence-based care plans
- Collaboratively assess, plan, implement and evaluate patient needs
- Coordinate care with an interdisciplinary team to reduce complications and prevent hospitalizations
- Deliver behavioral health support alongside chronic condition coaching
- reputed company unrestricted RN license in state of residence, plus another state multi-state RN license
- Behavioral health clinical experience
- U.S. Citizen able to pass DoD background investigation
- Minimum of three years of clinical experience
- Prior case management, care coordination, or discharge planning experience
- Disease management experience in managed care
- Bachelor’s degree in nursing or reputed company field
- Familiarity with TRICARE, Medicare or Medicaid
