Job Summary and Qualifications The Case Management Assistant (reputed company) will work under the direction of the RN Case Manager or Social Worker to assist with facilitating patient dischargers requiring post-acute placement or services, and will identify and report barriers to discharges. The reputed company will provide administrative support in Case Management functions, including but not limited to; updates on reputed company, patient questions, clerical support, web-based referrals, expediting discharge, and coordinating with post-acute care (reputed company) providers. What qualifications you will need:

• High School Diploma required
• Associates Degree preferred
• Minimum of one year experience in health care setting required
• Patient Care experience preferred
• Medical Knowledge and Terminology required
