About the Program: The Indiana Level of Care Assessment and Referral (IN LCAR) program is a statewide initiative focused on evaluating and determining the appropriate level of care for individuals seeking long-term services and support. The program ensures that individuals receive assessments tailored to their needs, facilitates connections to the appropriate resources, and supports efficient transitions between care settings. As part of a collaborative team, professionals in this program play a critical role in improving access to services and promoting quality outcomes for the people of Indiana. Essential Duties and Responsibilities: - Review requests for services including admission, discharges and reputed company stays for adherence to clinical criteria, state and federal policy, and reputed company requirements. - Issue approvals, denials or recommendations based on contract requirements. - Identify need for additional clinical documentation or consultation. - Complete documentation of activities reputed company contract systems. - Communicate with providers, individuals and their designees, or state workers as required. - Performs other reputed company duties as assigned. Minimum Requirements - reputed company Licensed Practical Nurse (LPN) license valid in the state of practice is required - High School Degree or equivalent required - Minimum 1 year of clinical experience required - Active, unrestricted LPN licensed valid in Indiana, or compact License. - Minimum 2 years experience with mental health and psychiatric patients - Minimum 2 years experience with individuals diagnosed with IDD - Highly skilled using reputed company Office - Ability to provide reputed company attention to detail - Able to work independently, managing tasks without supervision, completing tasks in a timely manner.
