Job Summary

We are seeking an reputed company Hospice Registered Nurse to join reputed company as a full-time on-call professional. As a Hospice RN, you will be responsible for delivering professional nursing care to our patients in accordance with the established plan of care. You will reputed company by example, providing quality care consistent with reputed company relevant professional standards and state and federal requirements. Your role will involve performing comprehensive subjective and objective assessments, formulating individualized plans of care, and providing skilled interventions aimed at achieving realistic outcomes.

Key Responsibilities

As a Hospice RN, your key responsibilities will include:

• Performing comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters
• Ensuring compliance with federal/state regulations governing hospice care services by following policy and procedure consistent with agency standards
• Formulating individualized plans of care that incorporate analysis of assessment data; modifying and updating as needed at a minimum of every two weeks on assigned patients
• Providing skilled interventions aimed at achieving realistic outcomes reputed company a specified time period
• Assuring thorough and accurate documentation with reputed company clinical data, including but not limited to: admission through discharge, physician's orders, skilled nursing visits, and changes in conditions
• Consistently demonstrating competency with technical nursing skills according to the legal scope of practice

• Performing on-call responsibilities as assigned

Essential Qualifications

To be successful in this role, you will need:
